Supported Living

Support Resolution works in partnership to support young people on the pathway to adulthood. 

The transition from Children’s Services into Adult Health and Adult Social or Learning Disability Services is an extremely important period in a young person’s life, requiring a multi-disciplinary approach.

Of course, the approach is person centred, in line with the young person’s rights, wishes and choices, and with a view to maximising their potential to reach goals and aspirations.

Supported Living

In the initial stages, Support Resolution offers its services including personal care and support, in the location identified by the Local Authority at the time of crisis or need.

Support Resolution do not own or rent any properties and so the offer of accommodation is not part of our service; however, we will assist young people and the funding authority to find a property that young people can make their own. We are building relationships with local Social Housing Associations and Specialist Supported Housing Providers that offer accommodation designed to meet the needs of the People We Support.

L       Life & Domestic Skills including support with budgeting, and benefit applications (PIP/UC) 

U       Understanding Social Norms and appropriateness 

T       Time management, including supporting with or the administration of medication

I         Inclusion in the community and support within family setting

O       Observation and Management of Health & Wellbeing 

N       New hobbies, interests and things to look forward to

Baseline assessment principles for moving from Children’s Services to Adult Services

The baseline assessment provided by the team consists of a comprehensive analysis of each young person’s skills, abilities, support needs and background, with the aim of deciding where support is best put in place.

Below you will find a list of topics covered in the baseline assessment process. Please note, each assessment will be person-centred, so the list is not exhaustive:

  • The young person’s experience of school and qualifications attained (if any).
  • Getting to know the young person, prior experiences, skills, likes/dislikes etc
  • Literacy and Numeracy skills (GCSE grade 4 achieved?)
  • Current support being received from LA, family, Social Workers etc
  • Current SEN status
  • Current housing status
  • Financial awareness
  • Independent living skills
  • Nutrition and wellbeing 
  • Independent travel skills
  • Current employment/education status
  • Looking ahead - long term goals, aspirations and progress towards these, including establishing possible issues around transition to college, Further Education etc.
  • Potential issues around safeguarding, keeping oneself safe and safety around others

Our Hybrid approach 

We understand that our Young People are at a critical time in their life. For many reasons they may not have had a stable loving family environment that in turn would lend itself to what might be considered a “Normal” routine of family life, development and schooling. Therefore, our Young People are at various stages of education and require support to understand what is required to give them the best opportunity to reach their full potential. 

Our qualified Education and Well-being Specialist uses a Baseline Assessment to work with our Young People to agree an Educational/Employment pathway and objective, in line with their educational status and aspirations.

Support Resolution is a Merseyside based family-owned company, established by a local family, to support local young individuals. A Supported Living provider for individuals (16 -25 age range) that may have a Learning Disability and or Autism, a physical disability, and young people that may have suffered a childhood trauma.
